Welcome Student-Athletes:
As
your sport season is approaching, many
details must be completed before
athletics starts in the area of Sports
Medicine.
The
MWSU Sports Medicine Office has made
available these forms and informational
material.
1)
MWSU Student-Athlete
Medical History Form
This form
must be filled out completely, signed
and returned.
2)
Insurance Letter,
Athletic Injury and Medical Policy,
and
Athletic Medical Bill
Procedure
These are
for your parents/guardians files at
home.
3)
MWSU Insurance
Questionnaire
This form must be filled out
completely, signed and returned.
4)
MWSU Athletic Department’s
Student-Athlete
Full Acceptance of Risk Form.
This form must be filled out
completely, signed and returned.
5)
MWSU Athletic Department’s
Student-Athlete
Consent Authorization
Form.
This form must be filled out
completely, signed and returned.
6)
Letter to Student-Participant,
Letter to Parent/Guardian, copy
of
MWSU Drug Policy, and
Letter of Consent to be tested.
The Letter of Consent must be
signed and returned, please keep the
POLICY for your files.
For
your safety and our protection, we will
require you to obtain a physical
examination from our Team Physician once
you are at Missouri Western State
University. This will give us a better
knowledge of your physical status and a
more complete picture of your medical
history.
Once
these forms are filled out completely
and returned to the MWSU Sports Medicine
Office we can set-up an appointment for
a physical examination with our Team
Physician.
